Vcare Urgent & Primary Care LLC and its affiliated entities (collectively, “Vcare Urgent & Primary Care LLC,” “we” or “us”) value our relationship with you and take your personal privacy seriously.
This Privacy Policy describes the type of information we may collect from you, how we might use or disclose it, how you can review or change the personal information you provide to us, how we may change this Privacy Policy, and the steps we take to ensure that your personal information is protected. This Privacy Policy applies to all users of our platforms and services, including Vcare Urgent & Primary Care LLC websites, mobile application, and other services (collectively, the “Services”), and to our practices for collecting, using, and sharing the personal information you provide to us in using the Services.
By accessing or using the Services or providing information about you to us in connection with them, you agree to the terms and conditions of the most recent version of this Privacy Policy.
Any health information considered “protected health information” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and that is created or collected about you by Vcare Urgent & Primary Care LLC in the course of using our Services is not subject to this Privacy Policy, but is instead covered by our Notice of Privacy Practices.
We may collect information that identifies, relates to, describes, references, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer, household, or device (“Personal Information”). We collect Personal Information when you use our Services, create an account with us, or submit Personal Information to us. The types of Personal Information that we may collect about you include:
- Information you provide to us. This includes identifiers such as your real name, address, birth date, email, phone number, or other similar information.
- For example, when you use our Services, such as speaking with a Vcare Urgent & Primary Care LLC employee over our online chat service, we may collect your name, location, email, platform, browser, entry page, language, IP address (the internet address of your computer), who referred you, and a record of the chat transcript.
- Additionally, when you sign up for an account with Vcare Urgent & Primary Care LLC we may collect your name, address, phone number, title, birth date, gender, together with other information such as occupation, industry, and personal interests. From time-to-time, we may also invite you to participate in surveys or promotional activities, such as running events. When you participate in these sorts of activities, we request certain Personal Information such as name, email address, phone number, where you work, and what Services you are interested in receiving. Depending on the nature of the activity, we use this information to follow-up with the participants, or, if applicable, to notify winners and award prizes.
- When you chose to sign up for or invite your family members to sign up for our CHET Community feature, we request that you provide certain information such as your name, email address and the name of the patient.
- For certain employees, clinicians, consumers and referral sources of Vcare Urgent & Primary Care LLC, we may collect your name, gender, marital status, birth date, partner name, practice location, title, email, phone number, fax number, address, occupation, specialty, NPI, hospital affiliation, your education and work history, credentials, and behavioral information that we infer about you. This helps us track our various interactions, such as where are you located and did you make an appointment.
- Communications from you. When you use our Services (for example on our website), complete electronic forms, or contact us, by online chat, email, phone, or text, we may automatically collect and store certain information about you and the activity you engaged in, for example: your name, address, the nature of your communication, the purpose of the interaction, and the action we took in response to your inquiry or request.
- Information related to your use of the Services. We may automatically collect information about your use of the Services, including information sent by your mobile device. For example, we may collect device information, such as your hardware model, IP address, unique device identifiers, and other information such as your browser type, operating system, and your website or mobile app usage.
- Information from your mobile device. We may collect certain information that your mobile devices send when you use our Services, such as a unique identifier, user settings and the operating system of your device, as well as information about your use of our services on your mobile device. We will request your mobile’s camera permission to scan your driving license during the registration process for your convenience.
- Information from our affiliates and partners. We may also receive your Personal Information from other Vcare Urgent & Primary Care LLC affiliates and subsidiaries, our business partners, service providers and other third parties in connection with one or more business purposes, including making our Services available to you
We may also obtain Personal Information from the following categories of sources:
- Cookies. Our Services may use cookies and other technologies to generate various reports on the traffic coming to our web properties to study user behavior, ways to improve our websites, our conversion metrics, and the effectiveness of digital marketing campaigns. We do not link this automatically collected data to other Personal Information we may have about you. A cookie is a small string of information that the website you visit transfers to your computer for identification purposes. Cookies can be used to follow your activity on the website, and that information helps us to understand your preferences and improve your website experience. Cookies are also used for such activities as remembering your user name and password, if you use such a feature on this site. You can turn off all cookies, if you prefer not to receive them. You can also have your computer warn you whenever cookies are being used. For both options you must adjust your browser settings (i.e., settings in Chrome, Safari, Firefox, Internet Explorer, or another browser). There are also software products available that can manage cookies for you. If you choose to reject cookies, you may limit the functionality of the website you visit, and you may lose access to some of its features.
- Tracking and/or Analytics Services. To gauge the effectiveness of this website, we may collect information about our website users using tracking and/or analytics services. This information may include, among other things, your: gender, age group, interests, language, location, IP address, browser type, internet service provider (ISP), domain names, referring/exit pages, operating system, date/time stamp, and clickstream data. We use these tools to generate various reports on the traffic coming to our web properties to study user behavior, ways to improve our websites, our conversion metrics, and the effectiveness of digital marketing campaigns. We do not link this automatically collected data to other Personal Information we may have about you.
We will only use or disclose the Personal Information we collect for one or more of the following purposes:
- To provide you with our Services. We may use your Personal Information to fulfill or meet the reason you provided us with the Personal Information. For example, we may use general location information to improve and personalize our Services to you, such as using your Personal Information to respond to an inquiry from you or providing location-relevant information and Services to you. Additionally, if you activate certain services or additional features, such as the CHET Community, we will use your information to provide those services or features.
- For our operations and administrative purposes. We may use your Personal Information for the purposes of furthering Vcare Urgent & Primary Care LLC business, including creating, operating, delivering, maintaining, and improving our content, products, and Services. We may monitor how our users use our Services including, without limitation, time spent using our Services, pages visited, and content viewed. We may also use it to help maintain the safety, security, and integrity of our website, products and services, databases and other technology assets, and business or for testing, research, and analysis, including to develop and improve our Services.
- For marketing communication purposes. We may use your Personal Information to send you updates regarding existing products and services, information about new products and services, upcoming events, surveys, and other announcements or inquiries. Users who do not wish to receive such communications via email can choose to opt-out. All our email marketing communications will contain an “unsubscribe” function.
- To comply with a legal or regulatory requirement or to protect ourselves. Our Services are subject to certain laws and regulations which may require us to process your Personal Information. For example, we may process your Personal Information to comply with employment laws or as necessary to manage risk as required under applicable law.
We may disclose your Personal Information to a third party in the following situations:
- With our affiliates, business partners, and service providers. We may share Personal Information with other Vcare Urgent & Primary Care LLC affiliates and subsidiaries, our business partners, service providers and other third parties to whom you or your agents authorize us to disclose to enable them to perform functions on our behalf, such as email and marketing outreach services. These third parties are only permitted to share, store and/or use Personal Information for contracted business purposes.
- CHET Community. If you chose to use the CHET Community service, we will share certain information with the individuals that you invite and allow us to share it with as well as third party service providers that help us facilitate this service.
- In the Event of Merger, Sale, or Other Asset Transfers. We may disclose your Personal Information to evaluate or conduct a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all our assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which Personal Information held by us about you is among the assets transferred.
To comply with a legal or regulatory requirement or to protect ourselves. We may also share your Personal Information when we believe that such action is necessary to:
- Fulfill an enforceable government request;
- Conform with the requirements of the law or legal process;
- Protect or defend our legal rights or property, this website, or other users; or
- Protect your health and safety or the health and safety of our users or the general public.
Our Services may contain links to other sites that are owned or operated by third parties. We are not responsible for the content, privacy or security practices of any third parties. To protect your information, we encourage you to learn about the privacy policies of those third parties.
Links to other websites that are owned or operated by third-parties may be available as part of our Services to provide you with useful information and services. You may also see our logo on external websites where we have partnered with third parties to offer you additional information and services. Links that take you off our websites are clearly labeled. We are not responsible for the privacy policies or actions of those who control other websites and encourage you to look for and review the privacy policies of any website you visit.
Vcare Urgent & Primary Care LLC strives to use commercially reasonable measures to protect the Personal Information you share with us, but no data transmission over the Internet or data storage system is guaranteed to be 100% secure. Information that you share with us on the Services is protected using encryption through “Secure Socket Layers (SSL)” as it passes between your browser and this website. Additionally, only authorized persons within Vcare Urgent & Primary Care LLC are permitted to access your Personal Information. All authorized persons must abide by security, privacy, and confidentiality agreements and/or policies. If you have reason to believe that your interaction with us is no longer secure, please contact us immediately in accordance with the “Contact Us” section in this Privacy Policy.
We retain your Personal Information for as long as necessary to provide you with the Services, as required by law, or to ensure that we can perform legitimate business functions, such as accounting for tax obligations or audits for security purposes.
Our Services are not directed to children, and we do not knowingly collect Personal Information from children under the age 13. If we find out that a child under age 13 has given us Personal Information, we will take steps to delete that information. Where deletion is not possible, we will take steps to de-identify that information. If you believe that a child under age 13 has given us Personal Information, please contact us in accordance with the “Contact Us” section in this Privacy Policy.
This section supplements the information contained in this Privacy Policy and is intended to comply with the California Consumer Privacy Act of 2018 under Civil Code §§ 1798.100 to 1798.199, as amended, and any related regulations or guidance provided by the California Attorney General (“CCPA”) and the California Online Privacy Practices Act (“COPPA”). This section applies solely to all visitors, users, and others who reside in the State of California. Terms defined in the CCPA carry the same meaning in this section. The CCPA and COPPA provide California residents with the additional rights listed below. To exercise any of these rights, please follow the instructions listed in this section.
Please note, that the CCPA does not extend to medical information governed by the Confidentiality of Medical Information Act (Part 2.6 (commencing with Section 56) of Division 1) or protected health information that is collected by a covered entity or business associate governed by the privacy, security, and breach notification rules issued by the United States Department of Health and Human Services, Parts 160 and 164 of Title 45 of the Code of Federal Regulations, established pursuant to the federal Health Insurance Portability and Accountability Act of 1996 (Public Law 104-191) and the federal Health Information Technology for Economic and Clinical Health Act, Title XIII of the federal American Recovery and Reinvestment Act of 2009 (Public Law 111-5). Subject to these exceptions, California consumers have the following rights.
Right to Know. You have the right to know and see what Personal Information we have collected about you over the past 12 months, including:
- The categories of Personal Information we collected about you.
- The categories of sources for the Personal Information we collected about you.
- Our business or commercial purpose for collecting that Personal Information.
- The categories of third parties with whom we share that Personal Information.
- The specific pieces of Personal Information we collected about you.
- If we disclosed your Personal Information for a business purpose, a list of the disclosures made for a business purpose, identifying the Personal Information categories that each category of recipient obtained.
Right to Delete. You have the right to request that we delete any of your Personal Information that we collected from you and retained, subject to certain exceptions. Once we receive and verify you and your request, we will delete (and direct our service providers to delete) your Personal Information from our records, unless an exception applies.
We may deny your deletion request if retaining the Personal Information is necessary for us or our service provider(s) to:
- Complete your transaction;
- Provide you with the Services;
- Perform a contract between us and you;
- Detect security incidents, protect against malicious, deceptive, fraudulent or illegal activity, and prosecute those responsible for such activities;
- Fix our system in the case of a bug;
- Protect the free speech rights, including the free speech rights of you or other users, or exercise another right provided by law;
- Comply with the California Electronic Communications Privacy Act (Cal. Penal Code § 1546 et seq.);
- Engage in public or peer-reviewed scientific, historical, or statistical research in the public interests that adheres to all other applicable ethics and privacy laws;
- Comply with a legal obligation; or
- Make other internal and lawful uses of the Personal Information that are compatible with the context in which you provided it.
Other Rights. California Civil Code § 1798.83 permits users of our Services that are California residents to request certain information regarding our disclosure of Personal Information to third parties for their direct marketing purposes. Exercising these Rights. To exercise any of the rights described above, please submit a verifiable consumer request to us by either:

Only you, or someone legally authorized to act on your behalf, may make a verifiable consumer request related to your Personal Information. In order to verify your identity, we will ask you for certain information. We cannot respond to your request or provide you with Personal Information if we cannot verify your identity or authority to make the request and confirm the Personal Information relates to you. Any new Personal Information collected for the purposes of verification will be deleted as soon as practical after processing your request, subject to legal retention requirements.
Making a verifiable consumer request does not require you to create an account with us. However, we do consider requests made through your password-protected account sufficiently verified when the request relates to Personal Information associated with that specific account. We will only use Personal Information provided in a verifiable consumer request to verify the requestor’s identity or authority to make the request.
We are not obligated to respond to a “right to know” request from you more than twice within a 12-month period.
Response Time and Format. We endeavor to respond to a verifiable consumer request within forty-five (45) days of its receipt. If we require more time, we will inform you of the reason and extension period in writing. If you have an account with us, we will deliver our written response to that account. If you do not have an account with us, we will deliver our written response by mail or electronically, at your option.
Any disclosures we provide will only cover the 12-month period preceding the verifiable consumer request’s receipt. The response we provide will also explain the reasons we cannot comply with a request, if applicable. For “right to know” requests, we will select a format to provide your Personal Information that is readily useable and should allow you to transmit the Personal Information from one entity to another entity without hindrance, specifically through an encrypted email.
We do not charge a fee to process or respond to your verifiable consumer request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request.
Non-Discrimination. We will not discriminate against you for exercising any of your CCPA rights. For example, as a result of you exercising your CCPA rights, we will not:
- Deny you Services.
- Charge you different prices or rates for Services, including through granting discounts or other benefits, or imposing penalties.
- Provide you a different level or quality of Services.
- Suggest that you may receive a different price or rate for Services or a different level or quality of Services.
Do Not Track. We do not have the capability to respond to “Do Not Track” signals received from various web browsers.

Scope:
This Privacy Policy applies to the personal information of California residents who are employees, independent contractors, interns, volunteers, owners, board members, and other individuals who perform work for Company, as well as their dependents, emergency contacts, and beneficiaries, (collectively, “HR Individuals”) in their role as HR Individuals. This Privacy Policy informs HR Individuals about the categories of personal information Company has collected about them in the preceding twelve months as well as the categories of personal information that the Comply will collect about HR individuals in the future.
Except where the Privacy Policy specifically refers only to a specific category of HR Individuals, e.g., employees, this Privacy Policy refers to all categories of HR Individuals collectively.
“Personal information” means information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular HR Individual or household.
1) THE CATEGORIES OF PERSONAL INFORMATION WE COLLECT ABOUT YOU
- Identifiers, for example: real name, alias, telephone number, postal address, e-mail address, signature, bank account name and number for direct deposits, and photographs.
- Professional or Employment-Related Information, for example: educational institutions attended, degrees and certifications, licenses, work experience and previous employers, professional memberships and affiliations, union representation, seniority, training, employment start and ending dates, and job title.
- Compensation and benefits information for employees, for example: salary, bonus and commission, equity compensation information, hours and overtime, leave information, bank details (for payroll and reimbursement purposes only), benefits in which you may be enrolled, and identifying information for dependents and beneficiaries.
- Non-public educational information, for example: academic transcripts.
- Commercial Information, for example: business travel and expense records.
- Internet Activity Information, for example: Internet browsing and search history while using Company’s network, log in/out and activity on Company’s electronic resources, interactions with Company’s Internet web site, application, or advertisement, and publicly available social media activity.
- Sensory or Surveillance Data, for example: voice-mails, recordings of meetings or video-conferences, and footage from video surveillance cameras.
- § 1798.80: personal information described under Cal. Civ. Code § 1798.80 to the extent not already included in other categories here, such as bank account number for purposes of direct deposit or insurance policy number.
- Preferences, for example, hobbies and leisure activities, membership in voluntary/charitable/public organizations, and preferences regarding work tools, travel, hours, food for company events, etc.
- Characteristics of Protected Classifications Under California or Federal Law for employees, for example: race, age, national origin, disability, sex, and veteran status as necessary to comply with legal obligations and to support diversity and inclusion programs; disability, medical condition, and pregnancy, childbirth, breastfeeding, and related medical conditions, as necessary to comply with Federal and California law related to leaves of absence and accommodation; and marital and familial status as necessary to provide benefits to employees and for tax purposes.
- Sensitive identifiers, for example: Social Security, driver’s license, state identification card, or passport number.
- Biometric information, for example, a fingerprint for a biometric timeclock.
- Health information, for example: information about an on-the-job injury collected for workers’ compensation purposes.
- Personal communications: the contents of mail, email, or text messages on accounts or services not owned, or subscribed-to, by Company only for purposes of legitimate Company investigations and, to the extent required by law, with your authorization.
- Login credentials, for example: login credentials to an online account owned or subscribed-to by Company or as necessary to review personal communications as described in the preceding bullet point.
- Religion, for example if volunteered by an employee in support of the employee’s request for a religious accommodation.
- Financial account access information, for example: payment card information with CVV and other information as needed for the HR Individual to purchase from a Company store.
- Sex life: to the extent relevant and necessary for a legitimate Company investigation and only if permitted by law, e.g., as needed to investigate allegations of sexual harassment.
- Sexual orientation to the extent volunteered by the HR Individual, for example, for Company diversity and inclusion programs.
Note on emergency contacts, spouses or domestic partners, dependents, and beneficiaries named under certain of the company’s benefit plans (collectively your “Related Contacts”):
- Company only collects contact information about emergency contacts.
- Company may collect the following categories of personal information about spouses or domestic partners, dependents, and beneficiaries: (a) Identifiers; (b) Commercial Information if, for example, Company arranges travel for a dependent to attend a Company event; (c) Internet Activity Information if the individual uses Company electronic resources and web sites; (d) Sensory or Surveillance Data if the individual enters Company facilities; (e) § 1798.80 personal information, such as insurance policy numbers if the individual is covered by Company insurance or a beneficiary; (f) Protected Categories of Personal Information, for example, childbirth to administer parental leave, marital status to pay taxes, and familial status to administer benefits; (g) Health Information, for example, infectious disease testing when a Related Contact attends a Company event or if needed to administer a leave of absence for you to care for a Related Contact; (h) sex life, for example, date of newborn delivery to administer parental, Family & Medical Leave Act, or California Family Rights Act leave; (i) Login Credentials to a Company benefits portal or site; and (j) Sensitive Identifiers, for example, Social Security numbers for beneficiary designations; personal communications if communicating with a workforce member.
Note on inferring characteristics: Company does not collect or process sensitive personal information or characteristics of protected classifications for the purpose of inferring characteristics about the HR Individual.
2) THE CATEGORIES OF SOURCES FROM WHICH WE COLLECT YOUR PERSONAL INFORMATION
- You, for example, in your application, forms you fill out for us, assessments you complete, surveys you submit, and any information you provide during the course of your relationship with us.
- Your spouse or dependent with respect to their own personal information.
- Internally generated, for example, Company may generate performance ratings, evaluations, hours worked, and other information about you.
- Vendors and service providers, for example, law firms.
- Affiliated companies, for example, when an employee works on a cross-enterprise team.
- Third parties, for example, job references, business partners, professional employer organizations or staffing agencies, insurance companies.
- Public internet sources, for example, social media, job boards, public profiles, and other public online sources
- Public records, for example, court records, and credentialing and licensing organizations.
- Automated technologies on Company’s electronic resources, for example, to track logins and activity across Company network.
- Surveillance/recording technologies installed by Company, for example, video surveillance in common areas of Company facilities, global positioning system (“GPS”) technologies, voicemail technologies, webcams, audio recording technologies, and blue-tooth technologies, any of these with consent to the extent required by law.
- Labor processes, including collective bargaining processes, National Labor Relations Board processes to determine union representation, and grievance and arbitration processes.
- Union agreements, including collective bargaining agreements and other agreements between Company and a union.
- Government or administrative agencies, for example, law enforcement, public health authorities, California Department of Industrial Relations, Employment Development Department.
- Acquired company, if Company acquired your employer, Company might collect personal information from that employer.
Note: This Privacy Policy does not cover background screening conducted by third-party background check vendors subject to the federal Fair Credit Reporting Act. Company provides separate notices for such screening.

2. Purposes Specific To Certain Categories Of Employees’ Personal Information

Purposes For Using Employees’ Biometric Information:
- Company uses biometric technology for security purposes to verify the identity of an individual entering certain Company facilities.
- Company may use biometric time clocks to help track hours worked by non-exempt employees.
- Company uses biometrics technology for security purposes to verify the identity of the individual entering areas where pharmaceuticals are stored.

- Company may use information about an employee’s sex life only to the extent relevant and necessary for a legitimate Company investigation and only if permitted by law, e.g., as needed to investigate allegations of sexual harassment.

Purposes For Using Employees’ Protected Categories Of Information:
Company collects information about race, age, national origin, disability, sex, and veteran status as necessary to comply with legal obligations, including the reporting requirements of the federal Equal Employment Opportunity Act, the federal Office of Contracting Compliance Programs (applicable to government contractors), and California’s Fair Employment and Housing Act. Company also collects information about disability status to the extent an employee may need special assistance during emergencies from Company or from first responders.

F. Deidentified Information
At times, Company converts personal information into deidentified information using reasonable measures to ensure that the deidentified information cannot be associated with the individual (“Deidentified Information”). Company maintains Deidentified Information in a deidentified form and does not attempt to reidentify it, except that Company may attempt to reidentify the information solely for the purpose of determining whether its deidentification processes ensure that the information cannot be associated with the individual. Company prohibits vendors, by contract, from attempting to reidentify Company’s Deidentified Information.
4) HOW WE MAY DISCLOSE YOUR PERSONAL INFORMATION
Company generally maintains information related to its personnel as confidential. However, from time to time, Company may have a legitimate business need to disclose personnel information for one of the purposes listed in Section 2, above, to one or more of the categories of recipients listed below. In that event, Company discloses your personal information and/or sensitive personal information only to the minimum extent necessary to achieve the purpose of the disclosure and only if the disclosure is permitted by the CPRA and other applicable laws.
- Service providers and contractors: Company discloses your personal information to service providers and contractors to assist us in meeting our business needs and contractual and legal obligations.
- Company discloses your personal information to service providers and contractors only subject to written contracts in compliance with the CPRA and any other applicable law.
- Service providers and contractors include auditors, administrative service providers, law firms, travel agencies, benefits providers, and any other entity providing services to Company.
- Affiliated companies: Other companies within the Vcare Urgent & Primary Care LLC family of companies.
- Clients and customers: This may include, for example, disclosing a sales representative’s contact information with clients.

a. Your California Privacy Rights
Subject to applicable law, HR Individuals have the following rights:
- Right to Know: You have the right to submit a verifiable request for copies of specific pieces of your personal information collected in the preceding 12 months and for information about the Company’s collection, use, and disclosure of your personal information during that same 12-month time period. Please note that the CPRA’s right to obtain copies does not grant a right to the whole of any document that contains personal information, but only to copies of “specific pieces” of personal information. Moreover, HR Individuals have a right to know categories of sources of personal information and categories of external recipients to which personal information is disclosed, but not the individual sources or recipients. Company does not always track individualized sources or recipients.
- Right to Delete: You have the right to submit a verifiable request for the deletion of personal information that you have provided to Company.
- Right to Correct: You have the right to submit a verifiable request for the correction of inaccurate personal information maintained by Company, taking into account the nature of the personal information and the purposes of processing the personal information.
- Right to Opt Out of Sale and Sharing: As noted above, Company does not sell personal information or disclose personal information for cross-context advertising.

c. How We Will Verify Your Request:
The processes that we follow to verify your identity when you make a request to know, correct, or delete are described below. The relevant process depends on how and why the request is submitted.
If you submit a request by any means other than through a password-protected account that you created before the date of your request, the verification process that we follow will depend on the nature of your request as described below:
- Requests To Know Categories Or Purposes: We will match at least two data points that you provide with your request, or in response to your verification request, against information about you that we already have in our records and that we have determined to be reliable for purposes of verifying your identity. Examples of relevant data points include your mobile phone number, your zip code, or your employee identification number.
- Requests To Know Specific Pieces Of Personal Information: We will match at least three data points that you provide with your request, or in response to our request for verification information, against information that we already have about you in our records and that we have determined to be reliable for purposes of verifying your identity. In addition, we may require you to sign a declaration under penalty of perjury that you are the individual whose personal information is the subject of the request.
- Requests To Correct or Delete Personal Information: Our process for verifying your identity will depend on the sensitivity (as determined by Company) of the personal information that you ask us to correct delete. For less sensitive personal information, we will require a match of two data points as described in Point No. 1, above. For more sensitive personal information, we will require a match of three data points and a signed declaration as described in Point No. 2, above.
We have implemented the following additional procedures when verifying the identity of requestors:
- If we cannot verify your identity based on the processes described above, we may ask you for additional verification information. If we do so, we will not use that information for any purpose other than verification.
- If we cannot verify your identity to a sufficient level of certainty to respond to your request, we will let you know promptly and explain why we cannot verify your identity.
d. Authorized Agents
- If an authorized agent submits on your behalf a request to know, correct or delete, the authorized agent must submit with the request either (a) a power of attorney, signed by you, that is valid under California law; or (b) another document signed by you that authorizes the authorized agent to submit the request on your behalf. In addition, we may ask you or your authorized agent to follow the applicable process described above for verifying your identity. 5) HOW LONG WE KEEP YOUR PERSONAL INFORMATION
If Company hires you, the information collected about your during the job application process may become part of your personnel file and may be used to administer the employment relationship and for related reporting and recordkeeping purposes. Company will retain this application information for the entire duration of your employment relationship with Company and for as long thereafter as permitted or required by applicable law. Company makes its document retention schedule available to employees for review.
Company will retain information of applicants who are not hired for four (4) years after the record is collected. These records will be retained for our internal recordkeeping and reporting purposes in compliance with California Government Code § 12946. During that time, we may use your information to consider you for positions in addition to the position(s) for which you initially applied.

- Requests To Know Specific Pieces Of Personal Information: We will match at least three data points that you provide with your request to know, or in response to our request for verification information, against information that we already have about you in our records and that we have determined to be reliable for purposes of verifying your identity. In addition, we may require you to sign a declaration under penalty of perjury that you are the individual whose personal information is the subject of the request.
- Requests To Correct or Delete Personal Information: Our process for verifying your identity will depend on the sensitivity (as determined by Company) of the personal information that you ask us to correct delete. For less sensitive personal information, we will require a match of two data points as described in Point No. 1, above. For more sensitive personal information, we will require a match of three data points and a signed declaration as described in Point No. 2, above.
We have implemented the following additional procedures when verifying the identity of requestors:
- If we cannot verify your identity based on the processes described above, we may ask you for additional verification information. If we do so, we will not use that information for any purpose other than verification.

d. Authorized Agents
If an authorized agent submits a request to know, correct or delete on your behalf, the authorized agent must submit with the request either (a) a power of attorney that is valid under California law, or (b) a document signed by you that authorizes the authorized agent to submit the request on your behalf.
